This actually happened at a highschool in my area. They sent out a bunch of fliers with condoms stapled to them, after one of the administration discovered what happened, they had to recall and track down every last one.
Here in South Africa that actually happened when they started giving out government issued condoms with staples through the middle. Looked exactly like that.

Unfortunately its not at all funny!
This actually happened in South Africa with condoms distributed by the Department of Health or other active organisation in the HIV sector.
To add to the woes we have the case where defective condoms were distributed and the standards authority involved in some irregularity.
